By signing this agreement you acknowledge that you are providing a temporary home for a shelter pet. You realize that while we use our best judgment, we have no real way of knowing how this pet will be in your home. We are not responsible for any damage done by this pet.

You agree that you will inform us of any aggressive type behavior immediately so we may remove the pet from your home if necessary, and further evaluate the pet.

You agree to have the dog wear a Springfield Humane Society ID tag on its collar at all times.

You agree to never allow a foster dog to be off leash when it is outdoors, unless it is in a securely fenced in area.

You agree to have the foster dog reside in the home, not outside. The foster dog may NOT be left outdoors if you are not at home. This includes outdoor kennel runs.

You agree to never allow foster pet to be left unattended with young children or your own pets.

You agree to keep foster cats and kittens inside at all times, and to transport them using a pet carrier we provide.
